Circle of Excellence Program

Applications are now closed.

Support the efforts of your volunteers by participating in the Circle of Excellence program. Join us in celebrating the significant contributions of volunteers to OLTCA member homes.

The Circle of Excellence program was created to recognize the significant contributions of a volunteer’s service to OLTCA Member long term care homes across the province.

OLTCA member homes can select one volunteer from their home to receive a Circle of Excellence certificate recognizing their contributions to the long term care home.

This program is designed to recognize volunteers and is not a contest. All OLTCA member homes can participate.

Lifetime Achievement Award

NOMINATIONS ARE NOW CLOSED.

Let’s celebrate the significant accomplishments of extraordinary people living in our member long term care homes.

Introduced in 1986, the Lifetime Achievement award recognizes residents of OLTCA member homes for their noteworthy accomplishments and outstanding contributions to society and/or their long term care home.

The certificate and award program has evolved and continues to bestow honour, not only on certificate holders and award winners, but also indirectly on member homes and their staff.

Consider selecting a resident from your home and help us celebrate a truly remarkable group of men and women.

Quality and Innovation Awards

Award Categories
OLTCA believes that quality is a purposeful activity, requiring vision, leadership, teamwork, knowledge, creativity, and sustained action. The Quality and Innovation awards focus on the interplay of people, processes and products with awards in five categories:

  1. Quality Improvement Team of the Year

    The Quality Improvement Team of the Year award is presented to a task force, committee, work group, collaborative, process improvement team or other group affiliated with an active member home that has achieved significant improvement in one or more of the Health Quality Ontario publicly quality reported indicators.

    Who can apply: OLTCA active LTC home members.

    The winner will be awarded $1000.

  2. Innovation of the Year

    The Innovation of the Year award is presented to an active or Affiliate member that has achieved early results from change idea, research project or collaboration with other health care stakeholders that has a positive impact on residents, staff or the home.

    Who can apply: OLTCA active LTC home members.

    The winner will be awarded $1000.

  3. Workplace of the Year

    Workplace of the year is presented to an active member that demonstrates staff engagement, recognition, and a positive work place culture.

    Who can apply: OLTCA active LTC home members.

    The winner will be awarded $1000.

  4. Resident-Centred Home of the Year

    Resident-Centred Home of the Year is presented to an active member that demonstrates significant progress in creating a home-like environment, promoting a relationship-centred culture and advances the Residents’ Bill of Rights.  This award is nominated and selected by current residents.

    Who can apply: OLTCA active LTC home members.

    The winner will be awarded $2000.

  5. Best New LTC Product or Service

    Best New LTC Product or Service award is presented to a new or significantly improved product or service developed or launched by an OLTCA Commercial member, that is most likely to have a positive impact on long-term care if more widely adopted.

    Who can apply: OLTCA Commercial and Affiliate members.

    The winner will be awarded a complimentary booth at This is Long Term Care 2018.

Leadership Awards

NOMINATIONS ARE NOW CLOSED.

OLTCA’s Leadership awards recognize individuals who have demonstrated measureable achievements in improving the quality, culture and value of resident care and consistently demonstrated excellence through critical thinking and problem solving, modeling behaviours, leading change and innovation and linking knowledge to practice.

Applicants can apply for one of the three categories of leadership awards:

  1. Nursing Leadership Award
    (4 recipients total – two registered nurses and two registered practical nurses)

    Eligibility: A registered nurse (RN) or registered practical nurse (RPN) currently working in an active member home who has demonstrated excellence in leadership and advancing nursing practice in long term care.

    Each recipient will be awarded $2,500.

  2. Personal Support Worker Leadership Award
    (4 recipients)

    Eligibility: A personal support worker (regardless of title) currently working in an active member home who has demonstrated leadership in advancing the quality and culture of person-centred care.

    Each recipient will be awarded $2,500.

  3. Leadership Excellence Award
    (1 recipient)

    Eligibility: A staff member currently working in an active member home who has demonstrated his/her ability to champion transformational change for better care and outcomes for residents; sustain organizational learning and inspire others.

    The recipient will be awarded $2,500.

OLTCA Education Bursary

APPLICATIONS ARE NOW CLOSED.

Each year, OLTCA offers limited financial assistance to the staff of association member homes through its bursary program. Bursaries are awarded to staff who use the knowledge and skills gained through their commitment to continuous learning, growth leadership.

Successful bursary recipients will receive between $500 - $2,000.

Clinical Protocol Excellence Award

APPLICATIONS ARE NOW CLOSED.

The Clinical Protocol Excellence Award is presented to a registered nurse (RN) or registered practical nurse (RPN) currently working in an Ontario Long Term Care Association or AdvantAge Ontario member home who has demonstrated his/her ability to champion the implementation of best practice clinical protocols. Recipients have gained the practical skills that enable transformational change, personal growth and organizational leadership necessary to advance resident quality outcomes.

Five recipients will be awarded $2,000 each.

Applications will open in 2018.
